If you are a current, or new, CCVIM patient and you are having trouble feeding yourself or your family, please fill out this form. We have some people in the community who are donating food gift cards to help our patients who are struggling. No one outside the clinic will know.

(We are still not able to take food donations, but anyone can donate food cards.)
