ELIGIBILITY

CCVIM, a recognized 501(c)(3) non-profit, provides free non-urgent primary medical care for working and non-working uninsured patients with incomes 300% or below the Federal Poverty Guidelines. Incomes are verified annually through tax returns or pay stubs.

We see patients without any form of insurance or assistance.

We see patients in Pennsylvania from Columbia County and lower Luzerne County.

We only see patients 18 to 65 years of age to avoid duplication of  eligibility for Pennsylvania’s Children’s Health Insurance Program (CHIP) for under age 18, and Medicare programs for those over 65.

We see patients with appointments- no walk-ins.

We do NOT prescribe any narcotics at the clinic.

INCOME GUIDELINES:

Income guidelines change annually but the CCVIM 300% FPL guidelines for eligibility in 2022 are:

  • Individual:     below $38,640.00
  • Family of 2:    below $52,260.00
  • Family of 3:    below $65,880.00
  • Family of 4:    below $79,500.00
  • Family of 5:    below $93,120.00
